The story draws inspiration from Silent Hill 2, widely regarded as one of the most acclaimed entries in the video game series. James Sunderland (Jeremy Irvine), a grief-stricken man, receives a mysterious letter from his late wife, Mary (Hannah Emily Anderson), urging him to return to Silent Hill—a town where the boundaries of reality are twisted by guilt, grief, and trauma.

Compelled by love and haunted by his past, James embarks on a journey into the desolate, fog-covered town, hoping to uncover the truth behind the impossible message. However, what awaits him is a nightmare born of his own psyche, as the town begins to manifest his darkest fears and regrets. The deeper he ventures, the more Silent Hill morphs into a living nightmare, populated by grotesque monsters, warped memories, and the iconic Pyramid Head, a harbinger of punishment and suffering.
